After filling out all the requested documents I was then charged $30. for part of the exam that I thought was free, I noticed later that they had entered my provider as EYEMED when I had stated clearly on the forms that HUMANA is my provider.. Dilation was not performed either.

Dr. was very efficient and to the point. However, I didn’t get a full prescription as my bi-focal add was missing. So I called many times all going to a no-voicemail and no one answered. The lack of communication with this location and the prescription mistake is not good. The person fixing the error did not apologize or have an explanation.

After making 2 appointments I found out they don't take my insurance for the eye exam. Wanted to give me a $125 deal for the exam and then they would take the insurance for lenses and frames, Thankfully they let me know before the exam. Needless to say I just left.
