This is what it says on the receipt you get when you pay for your food. I went to the website and a screen came up that said this page was no longer there. It directed me to go to the Subway website and complete it there. Well I couldn't find it there anyway. So if you read on your survey that you can take a survey and get a free cookie don't bank on it!
