A formal complaint about unacceptable food safety practices to DC Department of Health Division of Food and the Chipotle Mexican Grill corporate headquarters.
This complaint includes rude manager behavior, poor food safety actions by a line worker, and unsanitary conditions observed at the Chipotle located at 1045 5TH ST NW WASHINGTON, DC 20001: Ward 6 | Quad: NW. The incident occurred on the 20th of October, 2025. The facts below require immediate investigation, corrective action.
Incident Details
• While waiting in the service line, I observed a line worker repeatedly violate basic food safety protocols. The employee used a gloved hand to wipe food from the metal counter and push the debris onto the floor, then continued assembling the next order without changing gloves.
• The same employee reached into a bag of soft tortillas with that same gloved hand.
• While assembling a bowl, guacamole fell onto the metal counter. The employee used the same gloved hand that had just contacted the dirty counter and the bagged tortillas to push the guacamole back onto the food in the bowl.
• When another customer raised a concern about these clearly unsanitary practices, the manager on duty, identified as Taliah Swann, responded by yelling at the customer for placing a hand on the glass separator instead of addressing the food safety concerns.
• The manager refused to investigate or discuss the customer’s concern, denied service to the customer, and instructed the customer to leave the restaurant without attempting to resolve the issue.

Direct cross-contamination risk from handling ready-to-eat food with gloves that had contacted contaminated surfaces and a bagged product.
Failure to follow basic glove-change and surface sanitation procedures required in any food-service environment.

Review managerial conduct and, if warranted, take disciplinary or corrective action regarding the conduct of the manager on duty, Taliah Swann.

CLOSE THIS DOWN that’s all i can say THE worst chipotle in Dc. The service is awful. The food is either cold or overcooked all the time I’ve been coming to the chipotle for the last five years. The last two years has been horrible. The management is rude and never smells clean in there compared to the rest of the chipotles in the city. The one is barely getting by corporate needs to handle this immediately.
