Avalon Care Center at Northpointe

Avalon Care Center at Northpointe offers both short-term and long-term placement for patients recovering from either an accident or injury requiring skilled nursing and/or skilled therapy care. We offer a 20 bed sub-acute unit with a committed team of nursing and physical, occupational and speech therapy professionals. We have a dedicated Medical Director and supporting medical staff that provides medical supervision and assessments at bedside. This dedication provides the patient with a continued focus on recovery and well-being while at Avalon Care Center. We provide the care and services necessary to help the patient make a successful transition home.
