As a non-profit, Managed Care Organization (MCO), Lakeland Care delivers world-class service by providing long-term care supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program. Our goal is to help our members live as independently as possible because we know how important it is to remain connected to family, friends, and community. Lakeland Care supports members living in twenty-two counties and two tribes in Northeastern, Northern and Central Wisconsin. Lakeland Care offices are open by appointment.
