Do not send loved ones there if they need constant care. My father died of Sepsis and Infections due to neglect. Simply not enough staff to be considered a skilled nursing facility.

I had totally knee replacement in Dec if last year. I came here not knowing what to expect as my previous PT experiences were horrid. This has been a WONDERFUL experience for me. My therapist, Allan, was very patient and explained why each exercise was needed to regain full motion/strength. My sessions were productive and my surgeon was thrilled w/my early progress and stated my therapist was to thank. So, if you want good experienced PT, then I highly recommend Allan at the Montello Care Center. You won't regret it.

TERRIBLE facility! As someone who tries to promote growth and professional success, I can say that THIS facility NEEDS to be SHUT DOWN!! I had taken an opportunity to get a tour of the place for one of my loved ones and was left with an absolute DISTAIN for this facility. As I was walking with one of the staff members providing the tour, they said that they had an "important" phone call and that it would be quick. while I was waiting (about 12-15 minutes) I noticed an UNSUPERVISED RESIDENT who was seemingly wheelchair-bound, had gotten up and started walking away from the chair. When this resident had gotten up I noticed she was almost COVERED waist down in URINE! Unsure as to what I should do in that situation I immediately looked for another staff member to let them know. After peaking my head into a few empty offices, I noticed a nurse sitting down on her phone and told her what I had seen. She DARED to tell me that it's "A common issue" and that "They would get a shower later tonight anyways." I don't know who that person was but I was heart wrenched after seeing this and knew then and there that I WOULD NEVER, send my family to this place!!! Aside from the seemingly lazy staff and care, it was shown even more so when I got to the dining area. I overheard a gentleman in the kitchen calling others "Dumb dumbs" and "Soft-headed (R-Words)". I understand this is a small town, and that not everyone is perfect, but THIS IS UNACCEPTABLE BEHAVIOR! I'm currently talking with current CNAs and RNs at other facilities trying to figure out how I can do something to protect these residents and shut this place down. And as I've looked deeper and talked with ex-staff of this facility, they've had 5-6 investigations in a year?! Including CPR given to DNR patients, money embezzlement from administrators, and neglect and careless supervision resulting in injuries. Shame on you Montello "Care" Center, for even allowing me to see how poorly you guys run. I DO NOT wish you luck on these upcoming consequences, and hope that you care for the people around you, better than you do (Unfortunately) than these residents.

My great grandfather was sent to this facility on hospice, and in the week he was there before passing I never once had a good experience when dealing with the faculty. I had once asked about the status of a medication, as I was put on the list by his POA to receive any medical information I want, and I was given misinformation. I was given an answer but ended up being the opposite. The nurse didn’t even go check her records just answered. And as this medication has been being given the last two days she should have known or at least consulted the MAR. Also on the day before he passed I took my children to go see him and what I walked into was not okay. He was in his bed askew and was pinched between the wall and the bed frame. His mouth was full of old food that hadn’t been cleaned out. I personally used his sponge sticks to clean it (I work as a CNA in another facility) And when asked the last time he ate I was told a couple days ago so that means it wasn’t done for days and for anyone like him that means he was at a higher risk for aspirating and should have been done after every time he ate. He hadn’t been shaved in days and had crusted who-knows-what all over his face. As a visitor I should not have to come in and take care of my family myself. I regret that my grandparents ever sent him there. I would never leave someone’s family member in the state of disarray that they did to him.
