Our licensed assisted living home in St. Michael, MN provides high-acuity care for individuals with complex needs, including stroke recovery, tracheostomy care, dialysis support, COPD, spinal cord injuries, GI tube, colostomy, urostomy, Hoyer lifts, 2 persons assist and chronic wounds. We also serve residents with behavioral health conditions such as PTSD, depression, anxiety, and bipolar disorder. Staffed by a Doctor of Nursing Practice, RN, and 24/7 care team, we offer nursing-home level care in a warm, personalized home setting for only 5 residents. We accept CADI waiver and Elderly waiver (EW) as well as private pay.
Our home is handicap accessible and we provide transportation for our residents.
