    This facility is a disappointment. Disorganized, communication from staff was very inconsistent. Response to concerns was very delayed. I would recommend families to visit in person before making a decision to have their loved ones stay here.

    We moved my mom, Laurice, to the guardian facility, and I was initially skeptical. However, after a few months of being actively involved in her stay, I’ve come to realize that it’s a fantastic place. The nurses and nurse aides are incredibly caring, loving, and always willing to help patients. Paul, the nurse, is particularly kind and attentive. Helen, another nurse, is incredibly kind and does her job exceptionally well. Belinda, the CNA, tries to keep things light-hearted and humorous while she works. Linda, another CNA, is amazing and loving. Lucile, the CNA, is very helpful. From top to bottom, Nancy, the director, and all the nurses are great. The facility is clean, and there are many activities offered. The food is acceptable, but I’m sure they’ll make improvements.

    My husband has been at The Guardian Center for several months since his stroke. They installed a phone for him when I initially requested it but it broke about a week later. They replaced it but this time didn't give him his own number, it just comes up the main number. So for the past 3 weeks I have been unable to reach him by phone. I cannot call to check on him when he's ill, or to say goodnight every night. He is lonely and miserable. I have called every single day for 2 weeks about fixing his phone line. The 2nd floor unit manager never answers her phone or returns my calls and nothing has been done to fix the situation. My husband and his roommate also have not had a functioning clock in over a month and have no idea what time it is at any point of the day. I have been calling and complaining about that for over a month! The aides are not very friendly and for some reason they lied to the nurse on duty and accused us of smoking in his room! We don't even smoke! This place does not care about its residents or their families. No one on the 2nd floor answers the phone after 7 pm so good luck checking on your loved ones at night if they don't have their own phone! Zero stars. Do not recommend.

    I would just like to give Paula a big thank you. I know you do some things your not even recognized for. Believe me I appreciate everything you do. You made what I needed to do so much simpler. Just wanted to say thanks from the bottom of my heart. Just know im sure there are people out there that feel the same way I do but have a hard time expressing it. So thank you for what you do from all of us.MARK

    This by far was the worst nursing home/rehab facility. My mom was in from October to December and she had an HORRIBLE experience ever there. She was in there for just surviving a stroke she couldn't walk or move her arm and the PT services she received was horrible. No communication with the family unless I initiated it there was days and weeks I would call to get update on mom condition and noone would respond back. The Cna had an bad attitude with everything, mom would go days without showering and when asked they would say they were busy and that we as family are welcomed to give her a shower ourself if we wanted. Dinner we would bring for the member would stay days in the fridge and not giving to the patients and eventually get thrown away by us. Several times my mom had falls at the facility after multiple times mentioned to the social worker Donna to get her an alarm for the wheelchair and the whole time she was there she never recv one. Jennifer the nurse stucked as well, she don't know how to professionally respond to emails. It would be a whole wk ans half to get a respond back from her.for 2 wks I was trying to contact Donna the social worker about an issue I had she never responded until 3 days she was suppose to resign with a stupid email excusing herself for not responding but Cleary telling me she has resigned and would assign my mom case to someone else and than she disappeared. The day finally comes when my mom asked deliberately to be discharged because she was unsatisfied with the service in which we should've done sooner. I go to pick her up and was suppose to meet with the discharge nurse who never came in to work that day because she had a death in the family. Instead I had a male charge nurse come and give me mom discharge meds and left the room with little to no information as to what I'm suppose to do now., no equipment was giving toy mom as promised before discharged I jad to fight for them to give her aleast a walker to she's able to walk around after I drop her off at home. Pt nurse apologized for the misunderstanding and told me she would be placing an order for equipment that my mom is qualified for and have it be deliver3d at the house. It's been 2 months and we still have not recv any equipment. This was by far the worst nursing home..giving I have worked for 2 my in life and they were all the best. Unfortunately they weren't on the list when I chose.
