    I have lived at Park Ave for a couple of years now, and although my goal is to eventually return to leave and live on my own, I feel blessed and grateful to have had a safe place where people genuinely care about me during this time. Tasha in the kitchen consistently goes above and beyond. She truly cares about the residents and takes pride in the quality of the meals she prepares. Her kindness and dedication do not go unnoticed. The Activities Department feels like family to me. Grace and Lisa have been a part of my journey for a long time, and they have always been supportive, compassionate, and caring. They work hard to keep the residents engaged and connected. When I first arrived, the rehabilitation team helped me regain my strength and independence. Thanks to their hard work and encouragement, I was able to get back to functioning the way I used to. While I have not needed rehabilitation services recently because I have maintained my progress, I remain grateful for the role they played in my recovery. The Social Services Department has also been an incredible source of support. She has helped me in countless ways and has worked diligently to make returning to the community a realistic possibility for me. I understand that the facility, like much of the healthcare industry, faces ongoing challenges and changes. Despite those difficulties, I believe Park Ave is a wonderful place. Whether it is a temporary home, a long-term home, or simply a place to help someone regain their independence and return home, the staff's dedication and compassion make a meaningful difference in the lives of the residents they serve. I am thankful for the care, support, and kindness I have received during my time here.

    This place is horrible!! Smells really bad anytime I come morning,noon or night! I found my aunt who I was visiting on the floor in her room. There was MO sense of urgency to get her up. I told the nurses at the nurses station she needed to use the bathroom twice.. I wait 40 min and left because no one ever came and I was so upset! Do not come here even if it is a last resort!😢😢😢🥵🥵

    My brother was recently transferred to this facility following a heart attack and subsequent memory loss issues. Despite being referred by another rehabilitation center specifically for treatment, he was with this facility for only two hours before I was told he would be discharged. I have several serious concerns regarding this encounter: The facility deemed my brother competent despite his significant short-term memory loss and disorientation regarding his location and the date. I asked for an explanation as to how this conclusion was reached. I was met with extreme hostility. Staff members hung up on me multiple times, refused to provide a timeframe for his transfer back to the hospital, and denied my request to speak with a manager. Furthermore, I was told a social worker I had just spoken with was "gone for the day" immediately after our conversation. The lack of compassion and the unprofessional communication I experienced during this stressful time are inexcusable.

    My friend was in here not sure if he still it because he went to the hospital and was there for a while I called the hospital and they said he wasn't there anymore, I asked if he went back to the nursing home were he was before and they said that they don't know, so I called a few times and every time I call no one answers so I have no idea if he went back or not

    My husband spent about 8 weeks at Park Avenue Health Center earlier this year. I won't say the place is perfect - the decor is tired and like all of these facilities there is never enough staff. But, the staff on the floor that do the hard work of caring for our family members were kind and hard working. Juliana, the social worker, is genuinely concerned for those in her care and worked hard to do the best by my husband. The therapy team were amazing. Pinal, director of therapy and a hands on PT, had Tim up and walking in no time after months of being bedridden. Occupational therapy had him back to feeding himself and doing ADL's. The speech therapist, kept a close eye on his needs and adjusted his diet and therapy as needed. To me, the care shown by the staff on the floor, Juliana, and the therapists, especially Pinal, outweighs how pretty the place is.
