PrimeCare Fullerton Health Center provides high-quality, affordable primary care for adults and children in Hermosa and across Chicago’s Northwest Side. Services include preventive care, routine checkups, chronic disease management, care coordination, and access to specialty services. This location also offers integrated behavioral health and perinatal care services. We welcome new patients regardless of insurance status and accept Medicaid, Medicare, and most private insurance plans. A sliding fee scale is available for uninsured patients. Our team delivers compassionate, culturally responsive care focused on prevention, wellness, and access to care close to home.
