Immanuel Pathways is the Program of All-Inclusive Care for the Elderly (PACE®). It is a healthcare program designed specifically for older adults who need a nursing home level of care but want to stay aging at home. With PACE®, participants have a dedicated Care Team to coordinate everything, so they can live comfortably where they are. PACE® participants get the kind of care and therapies they need while living at home.
PACE® provides comprehensive healthcare and social services to older adults (participants) still living independently. Most of the participants who are in PACE® are dually eligible for both Medicare and Medicaid. PACE® participants receive comprehensive, coordinated, customized care across various settings.
