To make an appointment or ask a question, please call 970-945-2840. We also have locations in Avon, Basalt, Glenwood Springs, and Gypsum. We have five School-Based Health Centers located at Avon Elementary School, Basalt High School and Middle School, Glenwood Springs High School, and Roaring Fork High School.
