PACE provides coordinated medical and social services to seniors over 55 who have chronic conditions and require regular medical support, yet wish to continue living in their own home. We offer primary care, therapy services, wellness programs, in-home care, daily activities, and transportation to and from appointments. For seniors with Medi-Cal and no share of cost, all St. Paul's PACE services are free.
St. Paul’s PACE Chula Vista opened in 2012 thanks to a generous gift from the Akaloa Foundation. The area welcomed St. Paul’s with opened arms because they recognized a significant need for medical and social services for low income seniors in Chula Vista.
