Afton Community Health Center, “The Road to Better Health”, provides quality primary health care for people in NE Oklahoma. Our staff is caring, professional, dedicated & experienced. Health services include; Women’s Health Exams, Well Child Exams, Immunizations, Physicals, Geriatric Medicine, and Initial Medicare Exams. Disease Management for: Diabetes, Hypertension, Heart Disease, Respiratory Care and COPD/CHF. We offer assistance to address behavioral health issues, such as: Anxiety, Depression, PTSD, Chemical dependence, Substance abuse, Adolescent behavioral issues, Grief, Suicide Prevention. A sliding payment scale is available for patients based on family size and income. Our site is covered through the Federal Tort Claims Act.
