  I don't even know where to start. From the moment my mother was taken there until the moment she left, it was a disaster. The medical transport driver warned us upon arrival. "You're going to have to advocate for her" There was no soap in the soap dispensers throughout the facility (daily). I checked every day after the first day she was admitted. Some staff would come into my mother's room smelling like they just smoked weed in the hallway. Rather than continuing her progress with therapy, they stuck in her a broken wheelchair and a diaper. We were told by the move-in coordinator that she would receive a walker. By the second day, we went and bought a walker for her to use. The only staff member that would return any calls was the therapy director. When I brought up issues to another member of staff, she tried to claim that I never reached out to anyone there. I provided her the time, date, and numbers of my calls that were unreturned. The facility looks nice, but the administration is the problem.

  Avoid This Facility – Chaotic Communication, Zero Accountability, and Unprofessional Staff I would give zero stars if possible. My experience with the administration at The Sarah Community was a nightmare during what was already a stressful time for my family. The "Friday Afternoon Crisis" After being repeatedly assured by social work staff that a long-term care bed was available for my mother, the facility waited until Friday afternoon at 2:39 PM to notify me she would be discharged the following Monday. This gave me less than one business day to find a new facility for a non-ambulatory patient. Systemic Dysfunction & Lack of Care There is a massive disconnect between departments. Social work told me multiple times a bed was reserved; Admissions flatly denied it. I was gaslit by admissions staff who asked if I "thought" I had a conversation about a bed or if it actually happened. They even hung up on me when I expressed frustration at their lack of help. I ultimately had to drive to the facility on a Saturday to physically grab referral paperwork because they failed to fax it to the new facility. No Accountability from Leadership I sent a detailed email (12/13/2025) documenting these systemic failures and the unprofessional conduct of the admissions staff to both the Facility Administrator and the President of DePaul Hospital. To this day, I have not received a single response. It is clear that the administration couldn't care less about the families they serve or the distress their dysfunction causes. Failure of Care The irony is that after moving my mother to a different long-term care facility, she improved so significantly that I am now moving her into assisted living. The skilled nursing care at The Sarah Community, which was supposed to help her recover, failed to do so. This is a faith-based institution that claims to care, but their internal communication is broken and their leadership is unresponsive. If you are looking for a collaborative, compassionate environment for your loved one, look elsewhere. They will leave you scrambling at the 11th hour with no support and zero accountability from management.

  I would NOT recommend this facility for anyone . They abuse and mistreat your loved ones . When having an issue then trying to address it they lie to you and ignore you . They are rough , hateful and neglectful to your loved ones. The Elderly abuse hotline has been called and they have been reported . Protect your loved ones and find them a different home .

  Jason has been amazing to work with. Moving mom in to her forever home is never easy but Jason has made it wonderful. Thank you for caring about my mom and being so helpful with this transition.

  Toured the Sarah Community recently. The BBQ was delicious and they had a cute bluegrass band playing. It is a beautiful community that is founded in faith. The atmosphere is down-to-earth, welcoming and pleasant. I love the new renovations! Their common spaces are plentiful and convenient. Chapel was happening while we toured and I was told they have daily mass or rosary. We are touring for Independent now, but I like that they have the other levels on site as well as we will need it in the future.
