I don’t recommend the St. Therese Transitional Care Facility!
My 85-year-old father was in the transitional care facility for 4 days after a hospital stay due to a fall. His experience at St. Therese was so bad, that at one point we just packed up his belongings and moved him out. The worst thing that happened to him is that a night employee (who was late in responding to his call button), was so physically rough with him that he was shell-shocked for the remainder of the stay. This happened the first night that he was there. After that, my 86-year-old mother felt like she needed to sleep there to make sure that it didn’t happen again. In addition to this, there were several times that the call button response times exceeded 15-20 minutes. That’s tough when you’re 85 years old and on diuretics. Finally, most of the staff that interacted with us were unfriendly. Except for a couple of exceptions, we really didn’t see any smiles.

My 85 yr old father stayed at St Therese for 4 days as he had hand surgery and couldn’t use a walker. We had a horrible experience and decided to break him out of there and care for him ourselves. Many times he pressed the red button for assistance to the bathroom and it took 15-20 minutes. We witnessed this ourselves and assisted him.
My mom even slept overnight as she didn’t trust the care It was degrading for him to not make it to the bathroom. In addition, one night he was handled very roughly during bathroom assistance and was traumatized when we returned in the morning. Also, the social worker treated us with disrespect when voicing our concerns by acting angry. The physical therapist talked in front of my dad showing no compassion stating his cognitive score was not good and we had to explain it was due to fact that his blood pressure was extremely low due to his heart medication. (When she evaluated him he was drowsy). They have no compassion for the elderly. Lastly, they forgot his dinner along with 3 other rooms. I had to go to the kitchen to get him some food leftovers. We encountered a couple of good nurses there but majority were terrible. I don’t recommend anyone to take there loved one there.

There were some confusing and inconsistent details in my planned move to Woodbury. Kathleen got every detail straightened out - she's a LIFESAVER!! Demonstrates Saint Therese's motto to the max!

The staff at St Therese has been wonderful. The location is central and the room is both nice and spacious. Mandy- social worker and Ellen- lead nurse are great!

Whereas we don’t offer dining packages, we have a variety of options available to meet the needs and preferences of community members. Assisted living residents may dine in our dining rooms, cafés or bistros or even choose to order a meal for delivery to their residences!

No. Each Saint Therese assisted living resident is billed monthly only for the rate of his or her specific apartment, the care services he or she receives for the month in question and any applicable fees relating to late/incomplete payment or damages.

When are nurses and physicians on site?
Saint Therese has compassionate caregivers present 24/7 for residents, including registered nursing assistants, licensed practical nurses, registered nurses and skilled support teams. Whereas physicians regularly visit our communities, residents are encouraged to continue seeing their personal providers. (We can also help arrange transportation upon request.)

What therapy is available while I stay in transitional care?
Ascend Rehabilitation, a division of Saint Therese, provides physical, occupational and speech therapy to address specific needs of individuals in transitional care. Prior to discharge, we offer a Safe-At-Home assessment with a comprehensive report of recommendations for equipment and services to assure a safe discharge plan. And once you’re home, Ascend Rehabilitation delivers fitness and wellness program membership and home-based services throughout the Twin Cities.
What if I don't feel ready to go home after your transitional care discharges me?
Sometimes we need just a little extra help, and that’s okay! Since we’re all unique, recovery time is different for each of us. That’s why Saint Therese has short-term respite stays available for up to four weeks. Respite guests stay in a fully furnished apartment and benefit from every amenity and convenience independent living offers. In addition, home care services may be added to guarantee you’re as comfortable as possible.
Since I'm only staying at Saint Therese for a short time, can I still attend chapel services or participate in activities?
Our transitional care guests are encouraged to take advantage of everything Saint Therese has to offer! Regular Mass, ecumenical services, scheduled events and activities are just a few of the many exciting things happening every day in our communities!
How is memory care different than traditional long term care?
The first thing you’ll notice is the secured entry to Saint Therese memory care neighborhoods, encompassed outdoor spaces and that the safety of all our residents is top priority.
Just like within traditional long-term care, employees serve memory care neighborhoods 24/7 but the provided care is highly personalized and specialized—and that requires a few more helping hands. Saint Therese memory care employees receive ongoing training and education so each are prepared to appropriately respond to dementia-related symptoms like agitation, wandering or sundowning.
In addition, scheduled memory care activities and events are designed to engage, stimulate and socialize residents living with moderate to late-stage dementia.
How do I know if my loved one needs memory care?
