I visit many rehab facilities around Jax and found this one to be top notch! Rachael at front desk so professional, helpful, and friendly. Cleanest facility also. Quan was mopping the 200 hall and was very attentive to ensure no visitors slipped as they exited the patient’s rooms. Staff exceptional. Highly recommend!
I have a friend staying at this facility for a week so far and I have visited her 3 times during her care stay. Every single staff member that I had interactions with, was kind, friendly and professional. Their aftercare case worker was absolutely wonderful. I mean, no one wants to be at a medical facility, but, I would definitely consider Woodland Grove if it was for myself. I noticed patients were varied in their mental and physical status from post-stoke, post-surgery to old age or simply needing to get stronger in their walking and sitting abilities. I commend the staff and had a very pleasant experience. Great job! It takes a special kind of talent to work in rehabilitation and I observed committed and caring vibes.

My father did a three day “incarceration” there before I couldn’t live with it and had him moved. Careless staff sitting around on their phones with dirty rooms, staff is completely void of compassion. Went to pick up his belongings, they stole his iPhone and had his roommates soiled underwear stuffed in my fathers box like they didn’t expect his valuables to be picked up. If you have an elderly loved one do not send them here, it was heartbreaking,
Wrote a report about missing items with not so much as a phone call back, Christmas was right around the corner so I’m sure one of their kids received a new phone.

My son Chris was in Woodland Grove for 2 weeks for PT and OT. All the therapist especially Charles did an excellent job with him. When he went in he was unable to walk because he was in the hospital for 6 weeks in bed. He came home walking very well with a walker. Everyone there the nurses and CNA’s were very respectful and helpful. It was a wonderful experience for him.

Absolutely horrible place. Under staffed. I was diagnosed with 4 UTI and was never even tested for them
No call lights. You had to wait until someone checked on you, the beds were from WWW 2 easily and made a horrible noise when you changed the position
PT did the best they could, not enough therapies for all of the patients. 30 minutes maybe 2 times a week with multiple other patients
No choice in food and very limited liquids to drink
Ratio of patients to staff was easily 50 or more to each CNA
My daughter had to complain to the nursing supervisor and superintendent, to get anything done
She requested that one of the staff members was taken off of my caregivers list
They left patients in their wheelchairs for hours and hours
Also if you were in diapers. You could be left in a soiled one for hours
Also there were only 2 doctors for the entire place and they only came in twice a week. So you might not actually see them
