After waiting a long time, just now on 10/10/2015, I finally got a call from the SF passport agency and they explained to me that the Loomis post office did not complete the form (DS-11) correctly. Agent ID, Facility ID, and signature of the post office person are all missing. Now, we have to go there and redo that form again. God knows when we get the new passport 😡
