I got the NJ voluntary form installation form instead of the mandatory form. This problem is still not resolved. Second due to my pulmonary sarcoidosis, I cannot blow through the device to get the car started. I was told no adjustment could be made unless you had a doctor's note. So I had the vehicle towed to my house until a recalibration of the device occurs. Call Intoxalock was useless no one could help me with these problem. All I got was a teleworker in another country who provided no resolution to the problem. Needless to say it was a bad experience.However shop who installed the device did a good job I have no complaints about them.
