  I injured my left ankle and leg in a slip and fall on ice January 27,2021 and received treatment at Bronson Hospital ER. I was diagnosed with a fractured fibula ( near my knee). I was sent for follow up with this clinic. At my first appointment I saw the NP Jordan. His notes from the visit were lacking details that were given during the visit. My condition was listed as chronic which is VERY INACCURATE. Whoever transcribed the notes needs to proofread their work. He did not physically look at the area I was complaining about and disregarded my complaints. Then I was scheduled to come back 5 weeks later. The location of the break was NOT X-ray after the initial at the ER. They did X-ray the ankle and again in notes said it was chronic. The notes were again VERY INACCURATE. This time my name was listed as BRIAN when it is Brenda. No clue what is going on, but I am disgusted. These files will be summoned by the company that I fell at. Very unprofessional. Yes I have had ankle surgeries etc in the past, but injured it when I fell. I don’t feel as if they are taking this seriously. I chose to follow up with my primary as well as my foot doctor and have gotten help from them.
