The doctor didn’t sign the certificate only the form which I cannot use without the certificate being signed as well. Called for over a week for it to be signed. First the system was down now the provider is out for a week so the physical needs to be completely redone. Ridiculous. A simple yet huge error that I’ve never personally had made but I won’t be returning to this site.
