They are making me take a form to my doctor to fill out for a depression medicine I am on to verify my doctor prescribed them to me. My doctor fills it out and tries to fax it, and then finds they cannot fax anything, because it fails to send to them. I even brought my meds to the Concentra the day I did my appointment and it has my doctor's name on it. I feel like I was discriminated against at this establishment a little bit. Not to mention it took 3 hours or so to finish me up. I'm very pissed off about what this place did. I literally just had a DOT renewal done in March and got a 2 year renewal. Only reason I went to this place is because I found a new better paying job. I will never come back to this location ever again.
