21st Century Rehab is a physical therapist owned company that was founded in 1995 by Steve and Michelle Cassabaum. Since that time, 21st Century Rehab has added four partners, all physical therapists who work in our clincs. 21st Century Rehab has seven outpatient clinics locations in Altoona, Boone, Carlisle, Indianola, Madrid and Osceola and have seven hospital partnerships with Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Greene County Medical Center (Jefferson), Knoxville Hospitals and Clinics, Story County Medical Center (Nevada), Van Diest Hospital (Webster City) and Wayne County Hospital (Corydon).
21st Century Rehab provides quality physical therapy, occupational therapy, speech therapy, sports medicine, and occupational health services to patients and communities. The entire staff is incredible! Prior to going I had been encouraged to get injections for my knee pain, and FD Physical Therapy helped me get to a point of no pain with movements I wasn’t able to before. They listen to you and incorporate your lifestyle into the solution.
