How does hospice care begin?
Hospice care typically commences after a formal request is made or when a patient's doctor provides a referral. Following this, a hospice representative will contact the patient or family within 24 hours to schedule an initial assessment. Once eligibility is confirmed by a nurse and physician, and the necessary paperwork is completed, hospice services can begin. In urgent cases, we can expedite the process to start care even sooner.
Is hospice available after hours?
Our hospice services are accessible 24 hours a day, seven days a week. After regular business hours, calls are directed to an on-call nurse who can respond promptly. Additionally, our chaplains and social workers are available as needed to provide comprehensive support at any time.
Will hospice care for my loved one in a nursing facility?
Hospice care can be provided wherever a patient resides, including private homes, assisted living facilities, independent living centers, and long-term care facilities. However, please note that we may need to have a pre-established agreement with the facility to ensure our care team can deliver services at that location.
