After calling Capital One customer service, I had the same "missing" parts on my form from both faxes. Either one person made the same mistake twice or two people made the same exact mistake. It is an issue I have seen with this branch (which, I believe, should be included in employee training if it's not already done so). I wouldn't have had to come in a second time or call customer service multiple times if they knew how to fax properly.
