- DSDM SJul 17, 20251.0Zero star. Mismanaged unprofessional nightmare facility. Staff turnover is extremely high. Very very very poor communication and follow through by ALL levels of staff. Aides are not trained properly. And nurses are understaffed in IL, AL, Nursing at Tapestries unit.

I asked to speak with Director of nursing and was told there wasn’t anyone in that position, then I was given a name. I have had a severely negative experience at this facility. Almost none of which had to do with NURSES, AIDES, PT, OT & Dining room, patient care staff !
