Social Model The Grace house is designed to provide a safe space for older adults. Through the social model we will support seniors who are 55 & over who are able to participate in activities of daily living (AOL's) independently, or with minimal prompting. The social model is focused on social interactions such as activities, meals, and companionship. Distinctive and Bold The program is not only to minimize risk of developing severe mental illnesses, particularly age-related cognitive decline, it is also to improve our senior's quality of life and increase their opportunities to socialize and stay connected with their communities through Music Therapy. Nutrition & Wellness Our professional staff will assist our guests with their physical and nutritional needs during their time at The Grace House Adult Day Program. i.e exercise sessions, planning daily meals, personal care reminders.
